Five Wishes living will presentation

Maple Point Supportive Living is hosting a free, public workshop on completing the Five Wishes living will form. It will be held at 6 p.m. on Nov. 1.

Five Wishes is the first living will that talks about personal, emotional and spiritual needs, as well as medical wishes. The program will have a healthcare, legal and spiritual panel on hand to answer questions. The panel will include: Christy Barre, APN, FNP, MSN; John W. Foltz, P.C., Attorney at Law; Lindsey Wieneke, LSW; Beth Warren, Kirby Ambulance Service; Buster Chumbley, Church of Christ.

A complimentary copy of Five Wishes will be provided for guests. A light supper will be provided, so an RSVP would be appreciated, but not required, by calling Maple Point at 217-762-6500. Maple Point is located at 1000 N. Union Dr., Monticello.
